Unfortunately, it is clear that the MOHs tend to interfere more than they support/evolve clinical practice in Canada. We have had some MDs wrongfully negated (due to an error on the part of the MOH), that the MOH have reversed once flagged.

• This form is for Doctors to refer their patients for OHIP/MSP/AHS-covered virtual visits with our Dermatologists.

• Patients require an email account & electronic device to access our services.

• All information is submitted and stored securely on our PIPEDA-compliant platform.

• Referred Patients are ONLY seen by our Board-Certified Dermatologists: view their Profiles here

• We can return the Consult Letter to either a fax # or secure email.

• Referrals for complex dermatoses/long-term care will be redirected back to the referring physician to refer to an in-person Dermatologist. As aforementioned, DermCafe has a limited safe virtual-only scope of care

• For certain care (e.g. cryotherapy, intralesional injections, etc), the patient may be referred to the most appropriate Dermatologist's physical office available within our Network. This is subject to availability, and the very limited reality of Canadian dermatology care that we are all working with

*DermCafé is a virtual dermatology clinic. If patient is uncomfortable with basic technology (email, webpages), DermCafé is NOT the right service for them, and they will have to wait for a traditional Dermatology appointment at an alternative in-person Dermatology office.

If you thinking that your patient may need Accutane, we kindly ask that you refrain from doing bloodwork for them. We find that most provincial laboratories are very disorganized, and when bloodwork is done at a different centre, it actually delays care at DermCafe, as it takes a long time to retrieve the results, without which we cannot start treatment. DermCafe is happy to completely manage Accutane patients, including bloodwork monitoring, and we strongly prefer to provide patients with our own DermCafe bloodwork form to ensure the results return to us in a timely fashion

If you have colour photo files to attach to your referral, please feel free to do so with the "ATTACH" button on the form. Otherwise, please kindly refrain from faxing photos as black-and-white blurry faxed photos provide no diagnostic assistance
